An exciting opportunity has arisen for a highly motivated Senior Pharmacist with a passion for high quality medicines optimisation to join our friendly, forward thinking, and patient centred pharmacy team at Somerset NHS Foundation Trust.
As a key member of our senior team, you will play a central role in shaping, developing, and delivering a comprehensive clinical pharmacy service. Working closely with the wider multidisciplinary team, you will contribute to exceptional patient care while supporting safe, effective, and efficient use of medicines across our organisation.
This role will enable you to provide a high quality clinical pharmacy service to a wide variety of inpatients, bringing your expertise to patients with diverse clinical needs within both our community hospital and mental health settings.
You will also actively support our clinical governance agenda, including incident reporting, service evaluation, and the development of policies and procedures–helping to ensure robust assurance and continuous improvement in the delivery of medicines optimisation across the Trust.

Your Responsibilities include
⢠The postholder will support the Clinical Pharmacy Leads in planning and developing defined aspects of the clinical pharmacy service to community hospitals and inpatient mental health units in accordance with national and local standards.
⢠They will contribute to clinical governance through incident review, guideline and policy development, and routine monitoring of service quality.
⢠The role includes leading or supporting medicines safety, quality improvement and research projects within community and mental health care.
⢠They will provide specialist pharmaceutical input for complex regimens, advise on high risk medicines, and contribute to polypharmacy reviews and deprescribing initiatives.
⢠They will participate in MDT meetings, deliver education to healthcare professionals, and support departmental CPD.
⢠As a senior member of the team, they will provide supervision, coaching and training to junior colleagues, support appraisals and performance development, deputisefor the relevant Clinical Pharmacy Lead where required, and work collaboratively with colleagues across disciplines to support service priorities.
⢠The post will be predominantly based in the west of the county, delivering clinical pharmacy services across Minehead, Williton and Burnham-On-Sea Community Hospitals, as well as the Wellsprings mental health site.
⢠As part of the role, the successful candidate will undertake weekly dispensary cover at Musgrove Park Hospital and participate in the acute site weekend and bank holiday rota.
You will have
⢠Assessed as suitable to act as Designated Supervisor for Foundation Year Pharmacists by GPhC
⢠Credentialed member of the College of Mental Health Pharmacy
⢠Postgraduate training in Mental Health
Requirement
⢠Masters degree in Pharmacy (MPharm) or equivalent
⢠Registered with the General Pharmaceutical Council (GPhC)
⢠Post graduate qualification in clinical pharmacy or mental health pharmacy (or working towards)
⢠Non-Medical Prescriber (or willingness to work towards)
