FULL JOB DESCRIPTION

Sussex Community NHS Foundation Trust seeks Health and Therapy Assistants for its Home First service in Brighton. This hands-on role supports people recovering at home after hospital stays through person-centred reablement care.

Your Responsibilities include:

• Supporting personal care, therapy exercises and health monitoring in people’s homes.

• Assisting with wound care, rehabilitation programmes and safe use of mobility equipment.

• Taking observations, recording progress and sharing updates with the multidisciplinary team.

• Encouraging recovery, confidence and independence in daily living skills.

Work Schedule

Part-time role within a 7-day service operating 8am to 8pm, 365 days a year.

You Will Have

• Compassionate, motivated and proactive approach to supporting recovery.

• Ability to work with nurses, therapists and paramedics in a team setting.

• Full UK driving licence and access to a vehicle for travel across the area.

Requirements

• Right to work in the UK is likely required

• Willingness to complete the Care Certificate and competency framework training.
