Join our dynamic, award-winning Virtual Ward Team at the Norfolk and Norwich University Healthcare Trust (NNUHFT) , where we are at the forefront of transforming patient care through remote monitoring and virtual support.
As a healthcare support worker you will play a vital role in supporting patients and their loved ones with remote monitoring equipment. Allowing them to recover and be treated in their own homes, and reduce their hospital stay.
You will work closely with the whole team to ensure patients receive safe, compassionate and effective care at home.
The post is based at the NNUHFT site.

Main duties of the job
Key responsibilities:- Provide education to patients on remote monitoring equipment- Monitor patient observations and escalate concerns accordingly- Provide remote support to patients via phone or video calls- Assist with equipment set up and troubleshooting- Document all care accurately using electronic systems- Promote independence and wellbeing in patients- Liaise with members of the multi-disciplinary team
