FULL JOB DESCRIPTION
We are seeking compassionate and dedicated Home Care Service Providers to deliver personalized in-home care to clients with diverse needs. This role offers an opportunity to make a meaningful difference in the lives of individuals requiring assistance with daily activities, health monitoring, and emotional support. Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Provide medication reminders and assist with medication administration following care plans
- Prepare nutritious meals tailored to dietary needs and preferences
- Monitor vital signs and observe patient conditions, reporting any changes to healthcare professionals
- Support clients with memory care needs such as Alzheimer’s or dementia through specialized techniques
- Operate assistive devices such as Hoyer lifts and manage safe transfers and positioning
- Maintain accurate documentation of care provided, adhering to HIPAA regulations
- Perform light housekeeping tasks including cleaning and laundry to ensure a safe living environment
- Support clients during outings or community activities as needed
- Follow all safety protocols including CPR and First Aid procedures when necessary
Skills
- Proven experience in home & community care, assisted living, or long-term care environments
- Knowledge of patient observation, monitoring vital signs, and managing care plans
- Familiarity with working with individuals with developmental disabilities, autism, or disabilities requiring specialized support
- Experience in dementia and Alzheimer’s care, memory care, hospice care, or working with seniors in nursing homes or assisted living facilities
- Ability to perform heavy lifting safely using equipment like Hoyer lifts when required
- Strong communication skills for effective interaction with clients, families, and healthcare teams
- CPR certification and First Aid training preferred; ongoing compliance with HIPAA regulations essential
- Compassionate attitude with patience for working with vulnerable populations requiring social work or direct support services
