FULL JOB DESCRIPTION
As a CHCP Falls Prevention Support Worker in Hull, you’ll assist integrated fracture liaison teams visiting homes/care settings to register patients, perform delegated nursing/therapy tasks, record information, support treatment programmes, and handle administrative duties preventing falls while building confidence/independence for complex needs clients within community interest company prioritizing colleague voice, diversity, and high-quality person-centered care.
Your Responsibilities include:
• Register patients + identify service beneficiaries.
• Carry out delegated nursing/therapy tasks within competencies.
• Record accurate patient information (electronic/paper systems).
• Observe/report condition changes supporting treatments.
• Assist administrative duties ensuring smooth service operations.
Work Schedule
Part-time community/home visits across Hull.
Flexible organising own workload within guidance.
You Will Have
• Confidence managing own workload + priorities.
• Willingness learning new clinical competencies.
• Empathy supporting vulnerable falls-risk clients.
• Organisational skills for accurate documentation.
Requirements
• NVQ Level 3 Health and Social Care (or equivalent).
• Good communication with physical/mental health needs.
• Basic IT skills + confidentiality understanding.
• Independent working + team collaboration capability.
