CARE Team (Complex, Assessment, Risk, Empowerment)

Be the advocate for independence. Deliver holistic, strengths-based support to adults in the community. You’ll assess complex needs, manage risk, and empower individuals to live safely and well. Build strong local networks and become a trusted presence in people’s lives.
