Reporting to the RGN and Clinical Care Co-ordinator, you will play a vital leadership role, ensuring the highest possible standards of relationship-centred, clinical, and dementia care. Your responsibilities include:

• Support and guide your care team to deliver exceptional, person-centred care at all times.

• Contribute to smooth, consistent service delivery across the shift, ensuring residents feel supported, safe, and understood.

Deliver Exceptional Resident Care

• Assist with ongoing assessment, planning, implementation, and evaluation of residents’ care.

• Ensure care plans are accurate, relevant, regularly reviewed, and reflective of residents’ individual needs.

• Support residents with meals and diets tailored to their requests and wellbeing.

• Take part in and encourage meaningful activities that promote engagement, purpose, and joy.

• Maintain complete confidentiality and uphold all regulatory, statutory, and company care standards.

Be a Trusted Clinical Support

• Communicate promptly with the RN or Clinical Care Manager regarding any clinical concerns.

• Assist with medication management in line with company and local policies—observing the five rights of medication at all times.

• Support the admission and discharge process with care and attention, including the organisation of residents’ belongings.

Champion Dignity, Respect, and Connection

• Build strong, positive relationships with residents and their families.

• Engage residents in conversation and meaningful experiences that reflect their interests, life stories, and personal choices.

• Respond with empathy and calm to residents in distress, ensuring their needs are met with compassion and sensitivity.

Work schedule 

Full time role

Requirement 

• providing care: 1 year (preferred)

• supervisory: 1 year (preferred)

• NVQ Level 2 Health & Social Care (preferred)
