FULL JOB DESCRIPTION

An exciting opportunity has arisen to join the Mental Health Services for Older People Community Team in Whitby.

The team support individuals and families within the community so you will work alongside a professional and caring multi-disciplinary team to provide high quality care to those accessing the service. Within this role you will develop a good knowledge of local services within the area and support individuals in the community and local care/residential homes. You will need to be able to communicate well with a wide range of individuals, adapting your communication style according to individual need in a responsiveand caring manner.

We are looking for people who can work relatively independently, however you will be supported regularly with supervision and support and professional development.

You will need to travel independently over a large geographical area and will require suitable transport.

Your Responsibilities include:

• To work within the policies, protocols and clinical procedures of the Trust and to promote competent working practice within a supportive multi-disciplinary team.

• To liaise with Mental Health Nurses, Occupational Therapist and Physiotherapy staff within the team and carry out assigned duties as a member of the multi- disciplinary mental health team.

• To be prepared to work on own initiative as necessary

• Promote equality for all patients and understand their health needs.

• Demonstrate an awareness of service user and carer perspectives on the provision of treatment and continuing care.

• Ensure accurate record keeping in accordance with Trust policy.

• Participate regularly in clinical supervision sessions.

• Liaise with representatives from associated voluntary and statutory organisations e.g. social care and facilitate good communication processes.

• Attend in-service training and other study / refresher days to meet individual and service needs.

• Participate in performance reviews.

• Maintains safety, privacy and dignity of all patients in the delivery of patient centred care, recognising and respecting differences including spiritual and cultural beliefs.

• Responsible for recognising the signs of patient harm, abuse or neglect, including poor clinical practice, identifying issues regarding the safeguarding who may be at risk and reporting all such concerns in accordance with Trust

Work Schedule

Full-time 37.5 hours/week with flexible working options.

You Will Have

• Ability to travel independently in accordance withTrustpolicies and service need

• Effective verbal and written communication skills

• Time management and ability to prioritise effectively

• Competency in ECG and venepuncture

Requirements

• Diploma in Health and Social Care level 2 or equivalent

• Care Certificate

• Key skills in Literacy and Numeracy level 2

• Successful completion of Trust’s full HCA Framework within 6-month probationary period

• Previous experience as a Band 2/3 HCA or equivalent in a care environment.

• Previous experience of working with people with mental/social health needs or learning disabilities
