FULL JOB DESCRIPTION
Three Clover Care is seeking compassionate Full-Time Care Assistants to provide person-centred support for people transitioning from hospital back to home. This role focuses on helping clients regain independence, delivering care plans, and supporting rehabilitation in a supportive, family-like team.
Your Responsibilities include:
• Supporting service users with personal care, medication administration, vital signs, exercise and rehabilitation support.
• Assisting with domestic tasks and liaising with healthcare professionals such as district nurses and pharmacies.
• Helping individuals regain confidence, independence and live safely at home.
Work Schedule
Shifts between 7:00am and 8:30pm, with full-time contracts up to 40 hours per week. Flexible arrangements are available, including zero-hour and part-time options.
You Will Have
• Full UK driving licence and access to your own vehicle.
• Ability to read and follow care plans accurately.
• Strong communication skills and the ability to work independently and as part of a team.
Requirements
• Domiciliary/home care experience preferred, but full in-person training is provided.
• Basic IT skills for documentation and reporting.
• Flexibility to work varied shifts to meet service user needs.
