FULL JOB DESCRIPTION 

We are looking for compassionate and dedicated Support Worker with strong communication skills to join our expanding Outreach team.

 As an Outreach Support Worker, you will provide personal centred support to individuals with a range of needs, including daily living tasks, accessing the community, emotional support, and promoting independence. This role is ideal for someone who values flexibility and truly enjoys making a difference

Description:

-Provide practical and emotional support to service users in their homes and communities
– Support with daily living activities including shopping, appointments, social activities, and personal care
– Follow care plans and risk assessments
– Ensure safeguarding and health & safety policies are always followed
– Work collaboratively with families, professionals, and wider support teams
