FULL JOB DESCRIPTION
We are seeking an experienced and compassionate Care Worker/Coordinator/Team Leader to join our team. As a Team Leader, you will be responsible for providing high-quality care services to our clients while also coordinating and managing the care team.
Responsibilities:
- Provide direct care and support to clients, including assistance with daily activities such as personal hygiene, meal preparation, medication management, and companionship.
- Conduct initial assessments of clients’ needs and develop personalized care plans in collaboration with clients, their families, and healthcare professionals.
- Coordinate and schedule care services, including assigning care workers to clients based on their needs and preferences.
- Conduct regular check-ins with clients and care workers to ensure the delivery of high-quality care and address any concerns or issues that may arise.
- Train and mentor new care workers, providing guidance on best practices and company policies and procedures.
- Maintain accurate and up-to-date records of client care plans, progress notes, and other relevant documentation.
- Create and monitor rota
- Collaborate with healthcare professionals, social workers, and other stakeholders to ensure holistic care for clients.
- Participate in team meetings, training sessions, and performance evaluations as required.
- Serving as a team leader
Requirements:
- Previous experience working as a Care Worker and team leader or in a similar role with a minimum of 5 years experience
- Certification or training in caregiving, or a related field.
- Excellent communication and interpersonal skills, with the ability to build rapport with clients, their families, and care team members.
- Strong organizational and time-management skills, with the ability to prioritize tasks and manage multiple responsibilities effectively.
- Compassionate and patient-oriented approach to care, with a genuine desire to make a positive difference in clients’ lives.
- Ability to work independently as well as part of a team, with a flexible and adaptable attitude.
- Proficiency in basic computer skills, including the use of email, electronic health records, and scheduling software
- Up to date with all mandatory training
Expected hours: 38 per week
