FULL JOB DESCRIPTION

Nursing Direct is a specialist complex care provider delivering high-quality, person-centred care to adults, children, and young people with a wide range of complex needs. These include ASD, ADHD, PTSD, learning disabilities, complex physical health conditions, and mental health needs.

Due to growth in our paediatric complex care services, we are seeking experienced Clinical Nurse Assessors with paediatricexperience to join our team on a flexible, sessional basis.

You will work independently across the community, conducting detailed clinical assessments and supporting the development of tailored care packages for both adults and children, with a strong focus on paediatric complex care.

Your Responsibilities include:

• Visiting new and existing service users in their homes or community settings

• Completing comprehensive clinical assessments for new packages of care

• Supporting paediatric assessments, including children with complex clinical and behavioural needs

• Reviewing and updating risk assessments for existing clients

• Working collaboratively with the Clinical Lead to develop and review care plans

• Producing clear, accurate clinical reports following each visit

• Escalating clinical or safeguarding concerns appropriately

• Completing competency assessments for care staff (e.g.PEG, tracheostomy care, deep suctioning)

• Identifying risks and required interventions across physical, mental health, and behavioural needs

• Maintaining accurate clinical documentation including consent forms and care plans

• Acting as a clinical advocate for service users

• Representing Nursing Direct with professionalism and compassion at all times

Work schedule 

Contract: Flexible / Sessional (minimum 6 hours per shift)

You will have

• Experience: Nursing : 3 years (required)

Requirement 

• Registered Nurse (RGN, RMN, RNLD or RSCN)

• Essential: Experience working with children or young people in a clinical or community setting

• Desirable: Paediatric Nurse (RSCN) qualification or strong paediatric complex care background

• Minimum 2 years’ experience in community-based assessments

• Strong experience working with complex care needs

• Confident working autonomously in community settings

• Full UK driving licence and access to a vehicle

• Excellent communication and report-writing skills

• Sound knowledge of safeguarding (including children’s safeguarding), risk assessment, and evidence-based care planning
