FULL JOB DESCRIPTION

Key Responsibilities:

Care Coordination & Leadership:

  • Create, manage, and maintain efficient staff rotas for all clients, ensuring all care visits are covered seamlessly.
  • Act as the main point of contact for our team of care assistants, providing them with daily support, guidance, and leadership.
  • Assist with recruiting, inducting, and mentoring new care staff, fostering a supportive and skilled team environment.
  • Liaise confidently with health professionals (e.g., District Nurses, GPs) and family members to ensure a joined-up approach to care.
  • Hands-On Community Care:
  • Conduct initial assessments and onboarding for new residents/clients, ensuring a warm welcome and a smooth start to their care journey.
  • Provide hands-on personal care and support to clients in the community to cover visits, build relationships, and lead by example.
  • Perform regular spot checks in clients’ homes to monitor the quality of care provided.
  • On-Call & Operational Duty:
  • Participate in the on-call rota to provide urgent support and advice to care staff and clients outside of office hours.
  • Accurately maintain and update client care records, ensuring compliance with CQC standards.
  • Essential Requirements:
  • Must have a recognised Level 3 Diploma in Health and Social Care (or higher).
  • A minimum of 2 years of experience in a home care setting within the UK.
  • Proven experience in providing hands-on personal care is essential.
  • Full UK driving licence and access to your own vehicle.
  • Exceptional organisational and communication skills.
  • Important Notice Regarding Sponsorship:
  • We are a registered sponsor. This role is eligible for Skilled Worker visa sponsorship.
  • To be considered for sponsorship, candidates must already be inside the UK.
  • Ideal candidates will already be on the Skilled Worker route and seeking a new sponsor to enable career progression into a senior, coordinated role.
