Thames Reach is a homelessness charity based in London, specialising in helping people with complex and multiple needs, including those associated with poor mental health and substance misuse. We manage a range of services, including street outreach, frontline hostels, day services, specialist supported housing and employment and skills schemes. Last year we helped more than 12,000 people.
Role Purpose
The Assistant Support Worker is a member of the service delivery team, working alongside Support Workers and Lead Workers. This is an entry level role which will give the post holder opportunities to develop the level of skills desired to progress to a Support Worker.
They will:
Contribute to effective day-to-day service delivery.
Conduct Risk Assessments, Needs Assessments and move-on plans with clients in the service.
Liaise with external agencies and councils to ensure clients are receiving appropriate support.
Increase numbers of client engagements and moving out within a set timeframe
Ensure the environment is safe, healthy, and reflective of our ethos and supports our client’s aims.
