FULL JOB DESCRIPTION

The Prospering Place is a social care provider aiming to deliver high-quality person-centered specialist care in a semi-independent setting with tailor-made support packages for 16 to 21-year-olds. The purpose of this role is to successfully support young people between the ages of 16-21 to transition into independent living whereby they feel confident and well equipped with the necessary skills to live a self-sufficient life. The post holder will join our friendly and professional team and will be required to provide support services in line with the Prospering Place guidelines. We have a home in Hounslow but the role may include floating support services in the surrounding areas and we also have a children’s home nearby so its a chance to be part of these exciting developments

Key tasks and responsibilities

  • We expect all of our key workers to be passionate about making positive changes in young people’s lives, helping them achieve their goals and to aspire for positive outcomes.
  • Providing support with all aspects of the young people’s daily lives and encouraging them to engage with positive activities.
  • Encourage young person to access education, training, employment and/or purposeful activity supporting any application or enrolment process.
  • Help with CV writing and application forms for jobs.
  • Support our young people gain vital skills such as cooking, cleaning and maintaining good personal hygiene, shopping, budgeting.
  • Support young people with registering with a GP.
  • Support our young people to make important appointments including medical appointments and accompany the young people on those appointments. ·
  • Build positive relationships with external agencies and stakeholders including social services, education providers, voluntary organization’s, and to services that may provide support to young people such as drug and alcohol services, domestic violence support services, and voluntary organization’s.
  • Be able to lone work with young people and work autonomously as well as part of a team. ·
  • Prepare individual key working reports regarding each young person’s progress; these reports must be completed to a high standard for submission to the placing authority and other relevant partner agencies.

Skills required

  • Be passionate about working with young people
  • Be approachable and be a role model to young people
  • Willingness to support young people to help them transition into adulthood by assisting them with cooking, cleaning, hygiene, budgeting, finding jobs and education etc.
  • Able to develop positive relationships with young people, understand and meet their individual needs by developing support programmes tailored specifically for them
  • Able to use own initiative
  • Excellent oral and written communication skills
  • Be able to lone work with young people and work autonomously as well as part of a team.
  • Have good administrative and organizational skills.
  • Computer literate
  • Able to work flexibly
  • Applicants must be age 22 and above
  • Experiencing with young people/groups of people with challenging behaviours
