FULL JOB DESCRIPTION

The Population Health Management (PHM) unit is a system-supported initiative across Lewisham and Greenwich, working collaboratively with local partners. Lewisham Health and Care Partners (LHCP) serve as primary health and care commissioners and providers in Lewisham, while the Healthier Greenwich Partnership (HGP) unites health and care providers in Greenwich.

LHCP and HGP are jointly implementing a population health analytics solution, leveraging South East London ICB as their anchor institution. This role will support analytics for both Lewisham and Greenwich.

Providing system-wide leadership, LHCP and HGP drive health and care transformation across both boroughs. A key aspect of this is the Oracle Health Intelligence Data platform, which integrates local health data into a unified system, enabling professionals to better understand population health needs and develop integrated service solutions.

An exciting opportunity has arisen for a Population Health Data Analyst to join the Population Health and Care team within Lewisham and Greenwich ICS places. The analyst will play an integral role in the Analytics workstream, supporting innovative projects that drive change and enhance clinical effectiveness.

Lewisham and Greenwich are the only boroughs in South East London with a population health management information system capable of transforming care. As a programme, we have collaborated on various impactful projects, including:

Main duties of the job

Chronic disease management covers hypertension, diabetes (undiagnosed cases, non-diabetic hyperglycemia, gestational diabetes), CKD, atrial fibrillation, and COPD.

Population health profiling in Lewisham and Greenwich.
Tackling health inequalities, including Core20PLUS5, prioritizing health checks for vulnerable patients, and a weight management service for Black African and Caribbean communities.

COVID-19 Vaccination Delivery Group with the London Borough of Lewisham.

Prevalence of the Vital 5 in collaboration with King’s Health Partners (KHP).

Quality improvement projects using disease registers to identify at-risk and undiagnosed individuals.

This is an exciting time to join as we expand into data science and predictive modeling to identify those most at risk and drive impact.

The successful candidate will support clinicians and commissioners, translating key questions into data queries and producing analytics to inform decisions and improve health outcomes.

skills & experience:

Experience working with data in acute, primary care, community, public health, mental health, or social care.
Strong programming/analytical skills (SQL, R/Python, Tableau).
Excellent communication skills for conveying complex data insights.
Knowledge of information governance and data ethics.
Ability to work autonomously and manage workload.
Experience with EMIS in primary care is an advantage.

Key Result Areas & Performance:  

Work with the Principal Population Health and Care Analyst and Programme Manager to develop and implement the Population Health Analytics plan that will support clinical and commissioning workstreams    

Liaise with analytical teams in partner organizations to ensure local analytics staff are supported in their use of the PHM tools.

Ensure that local and national policies and procedures relating to data security, data sharing, analysis, and modeling are implemented and maintained in line with national doctrine.

Become a Subject Matter Expert (SME) in Population Health Analytics in Lewisham and Greenwich and support the rollout of analytical functionality across all partner organizations.

Working with the members of the Population Health Analytics Steering Group, develop and implement standards around data visualization and data delivery.

Work with our third-party partners and the Analytics Steering Group to agree and deliver analytical tools that are efficient to create and effective in supporting end users’ Population Health goals.

To perform any other duties deemed appropriate by the Analytics Workstream or Strategic Lead for Population Health Analytics  

  Workforce:  

 Work closely with, and provide peer guidance to, analysts employed by partner organizations to ensure that workloads are manageable, deadlines are achieved to an appropriate quality standard, and training plans are agreed upon where appropriate, etc..  

 Communications and Relationships: 

Engage with the analytics teams in other Population Health networks to share reporting tools and products and support national developments.

Develop a communication plan to ensure that clinical and care teams across partner organizations can access population health analytical support and understand the products of that analysis
