We are expanding our team and looking for a dedicated Care Coordinator to join us in making a positive impact.


As a Care Coordinator, you will be responsible for overseeing and coordinating the care provided to our clients.

YOUR MAIN DUTIES WILL INCLUDE BUT NOT LIMITED TO :

  • Managing a team of care staff and ensuring high-quality care delivery.
  • Conducting assessments and creating care plans tailored to individual client needs.
  • Liaising with clients, families, and healthcare professionals to ensure smooth service delivery.
  • Monitoring and maintaining records of care schedules and client progress.
  • Providing support and guidance to care staff, ensuring compliance with policies and procedures.
